Note: Models 22100FSM and 22111FSM were also sold as two packs with Models 93100FSM and 93111FSM. The outside packaging would have the 93 number; however the individual units are marked with the 22.
Production Dates: September 28, 2004 through September 20, 2007
Consequence: The elastic straps that secure the pad to the shell can come loose and be accessible to the child. The child could then loop the elastic strap around portions of his or her body possibly resulting in injury.
Corrective Action: Dorel will provide a free repair kit. Owners can contact Dorel at 1-888-698-6681, Monday through Friday 8am to 4:30pm EST or visit their website at Dorel Juvenile Group.

Defect: The harness adjustment strap can loosen while in use. In the event of a vehicle crash, the child may not be properly restrained, possibly resulting in an increased risk of injury.
Corrective Action: DJG will contact all registered owners and provide them with information on how to obtain a free repair kit to replace the harness adjustment strap. Those consumers who have not registered their product should visit Dorel Juvenile Group or call 800-219-0509 to order a free repair kit.

Defect: There is a potential for a child to have his/her finger stuck in the square hole located on the left armrest and may not be able to pull it free on their own possibly resulting in injury.

Defect: One or more of the six handle trim screws can loosen and fall out into the child occupant area. The child could then pick up the screw and place it into his or her mouth.
Corrective Action: The child could choke on a screw. Owners can obtain the repair kit by contacting DJG at 1-877-229-1374. The recall is expected to begin on November 21, 2006.

Defect: If the harness is not properly tightened, the infant seat can experience web movement or web creep. If web creep were to occur, in the event of a vehicle crash, the child could move too far forward, possibly increasing the risk of injury.
Corrective Action: DJG will notify its customers and provide a free in home remedy kit consisting of two clips and installation instructions free of charge. Customers can contact DJG at 800-881-0570.

Noncompliance: These seats fail to meet the 70 degree maximum back support angle requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." In the event of a vehicle crash, the child may not be properly restrained, increasing the risk of injury.
Corrective Action: Dorel Juvenile Group (DJG) will mail a warning label instructing the owner not to use the child restraint in the rear facing position. In addition, customers who desire a rear-facing child restraint will be provided, at no charge, a DJG TLC Model 93-000-FSM. For additional information concerning this recall, customers can contact DJG at 866-611-3022.

Defect: With an affixed label on the seat, DJG intended to recommend that the forward-facing mode should be used for children "over 1 year of age." However, the label was misprinted to recommend that the forward-facing mode be used for children "under 1 year of age." If the consumer were to follow the erroneous instructions, the seat may not adequately restrain the child in the event of a vehicle crash, possibly resulting in injuries to the child.
Corrective Action: Dorel Juvenile Group will mail a corrected label along with repair instructions to registered owners free of charge. Customers who have not registered their child seat should contact DJG toll-free at (866) 611-3022 to obtain the free repair kit.

Corrective Action: Dorel Juvenile Group will mail a free in-home repair kit along with repair instructions to registered owners. Nonregistered owners of these seats are advised to check the manufacturing date and model number to determine if their seat is affected by the recall. Corrective Action: Dorel Juvenile Group will notify registered owners about this recall. All owners are advised to check the manufacturing date and model number to determine if their seat is affected by the recall. If so, owners should contact Dorel by visiting their website at Dorel Juvenile Group, click on "consumer services" or calling Dorel's toll-free number (800-880-9435) to receive a free repair kit. Until the repair kit is installed, owners are instructed not to carry the seat by its handle.

Defect: When used as a carrier, the handle may slip from one position to another, causing the seat to fall forward. If the child is not properly buckled into the seat, the child can fall out if the carrier, possibly resulting in injuries.
Corrective Action: Dorel Juvenile Group will notify its customers and provide a repair kit along with installation instruction free of charge. Owners who do not receive the repair kit within a reasonable time should contact DJG at 800-536-1090.

Corrective Action: Cosco will provide the owners with a free repair kit which will make the handle stronger. Owners are also being advised not to carry the seat by the handle until the repair kit has been installed.

Noncompliance: When these seats were tested dynamically per the requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems," structural failure occurred in the belt path area of the shell in the forward facing reclined position only.
Corrective Action: Cosco will provide inspection instructions to the owner. If the seat is involved in the campaign, a remedy kit will be provided to the owner.

Defect: Separation of the buckle latch release button and spring from the latch release housing. The seat will become inoperative as the user will not be able to reinsert the spring and red latch release button properly into the housing.
Corrective Action: Cosco will replace the entire latch release assembly.

Corrective Action: Cosco will provide a retrofit kit to owners of these seats which will include an easy-to-install metal plate that hooks over the base of the seat and is securely snapped in place by two plastic fasteners.

Defect: In a vehicle crash, at speeds greater that 30 miles per hour, the buckle latch housing mounted in the base of the shell can deform, causing the latch mechanism to fracture, allowing disengagement of the buckle tongue from the buckle housing.
Corrective Action: Cosco will send to owners a replacement buckle housing which can be easily replaced by removing the four screws attaching the original buckle latch assembly.

	Forward Facing Car Seats A forward-facing car seat is one in which the child sits facing the front of the car. The seat should be installed in the back seat of your vehicle. Most forward-facing only seats are for children above 20 pounds.

	Rear Facing Car Seats A rear-facing seat is one in which the infant faces the back of the car. Always install a rear-facing seat in the back seat of your car. This is the safest position for an infant. NEVER place a rear-facing seat in the front seat of a vehicle with passenger airbags.

	Convertible Car Seats Convertible seats are for infants and toddlers. The seat can be used as a rear-facing seats for younger children, and switched to a forward-facing seat on the child's first birthday if the child weighs at least 20 pounds. However, experts recommend keeping the child in a rear-facing position until he or she outgrows the weight or height allowed by the safety seat.

	Infant Car Seats Infant-only rear-facing seats are for babies up to about 22 to 30 pounds, depending on the specific car seat. You will need a new seat when your child gets bigger. Infant-only seats have handles that allow you to move the seat from the car to the house or other locations. Some have a base that you can leave installed in the car, so you can just click the car seat into place each time you use it.

	LATCH LATCH stands for Lower Anchors and Tethers for CHildren. It is a system mandated by the Federal government in an effort to standardize and simplify the installation of child restraints without using the vehicle's seat belt system. New vehicles and child safety seats manufactured on and after September 1, 2002 will be equipped with the LATCH system. Cars, minivans, and light trucks will be required to have anchor points between the vehicle's seat cushion and the seat back in at least two rear seating positions, and a top tether anchor (with the exception of convertibles). Child safety seats will have a lower set of attachments that fasten to the vehicle anchors. Most forward-facing child safety seats will also have a top strap (tether) that attaches to the top anchor in the vehicle.
